  • Referrals for inadequate financial assistance for housing, food, and utilities
  • Parenting education and support
  • Advance Care Planning
  • Inappropriate use of medical care (missed appointments, frequent ER visits)
  • Medicare or Medicaid navigation issues (Area Agency on Aging and ECDHD)
  • Referrals for substance abuse, suicidal, mental health resources
  • Brief intervention for patients with depression/anxiety
  • Referral for transportation to appointments
